HIPAA NOTICE OF PRIVACY PRACTICES

We are required by law to:

  • Notify you following a breach of unsecured PHI

  • Follow the terms of this Notice

Treatment

To provide medical care, coordinate services, consult with other providers, or manage referrals.

Payment

To bill insurance, process claims, or verify coverage.

Healthcare Operations

Quality improvement, training, licensing, audits, and internal operations.

As Required by Law

We may disclose PHI when required by federal, state, or local law.

Public Health & Safety

Reporting diseases, preventing serious threats, or assisting law enforcement when required.

Workers’ Compensation

For work-related injuries or claims.

3. Uses Requiring Your Written Authorization

We will not use or disclose your PHI for:

  • Marketing

  • Sale of PHI

  • Most sharing with social media or advertisers

  • Psychotherapy notes unless you give written authorization.

    You may revoke authorization at any time.

4. Your Rights

You have the right to:

  • Request a copy of your medical record

  • Request corrections to your record

  • Request confidential communication

  • Restrict certain uses of PHI

  • Request an accounting of disclosures

  • Receive a paper copy of this Notice

  • File a complaint without retaliation

Complaints may be filed with our office or with:
U.S. Department of Health & Human Services (HHS)
Office for Civil Rights (OCR)

5. Patient Portal (Healow/EclinicalWorks)

Use of the patient portal is voluntary.
Messages sent through Healow are part of your medical record and protected by HIPAA.

PHI is never shared with:

  • Google

  • Meta/Facebook

  • TikTok

  • Analytics platforms

  • Advertising platforms

6. Changes to This Notice

We reserve the right to change this Notice at any time. Updated versions will be posted on our Website and available at the office.
